Weird Adsense Ad

I have just recently started using DFP and noticed this ad. The youtube link actually links to my site. Can anyone give me some insight please? See image below.

    I get those on occasion; my Google account manager spent days with their technical team trying to track it down and in the end they said it is entirely the result of human error on the part of an advertiser who input their ad incorrectly into AdWords.
