payee name and account holder name needs to be same?

Hello,
In Google Adsense account we cant change payee name but recently google has introduced wire transfer method for Indian publishers too.. So in that we need to enter account holders’ name.
Can we keep account holder name different than payee name (account holder name will be same as in bank account ) or we need to keep same names in both?
Policy Violation

I have just received a Policy Violation, the violation says “Template site”, is this because I run part of my site on Subdreamer and the Adverts are placed on every page of the site automatically.
Would I just be able to run the advertisements on the home page so they are not replicated throughout?I also run vBulletin, do you also know if it is ok to run adsense on there?
Made me totally worried sick, hopefully you can offer advice so that I am able to correct.
I have removed all Adsense from main site and left them just on the forum and don’t mind leaving it like that if then they will be reinstated.

Questions for the registration test
How is eCPM calculated?
- eCPM = (page impressions/1000) x CPC
- eCPM = [(page impressions x Coverage) / 1000] x CPC
- eCPM = (total earnings x 1000) / page impressions
- eCPM = (page impressions x 1000) / total earnings
- eCPM is based on bids of the advertisers and therefor is not calculated
One of the following methods IS allowed by AdSense, which one?
